By filling in this form, you’re giving us permission to share your story – along with any photos or videos you provide or that we create with you. You can change your mind and withdraw your consent at any time.

What I understand and agree to:

• Different Strokes will store my story, and any photos or videos of me, safely and securely.
• My story may be shared in:
  • Printed materials (for example, newsletters, fundraising appeals or adverts)
  • Online (for example, on the Different Strokes website or social media channels)
  • The media (for example, local newspapers or other press)
• My story will always be shared truthfully and respectfully – it will not be used out of context.
• If my story, photos or videos include other people, I confirm they know this and understand they may be published publicly.
• I can withdraw my consent at any time by contacting info@differentstrokes.co.uk or calling 0345 130 7172. If I do this, Different Strokes will stop using my story and remove my photos or videos wherever possible.
